To request your records:

  • Contact Peterson Chiropractic by phone, email, or in person.
  • Complete a Medical Records Request Form if requested.
  • Let us know whether you would like your records provided electronically or as a paper copy.

We will process your request in accordance with applicable state and federal law. Fees, if any, will comply with state and federal regulations.
